First Reconciliation/First Communion Registration Form - 2026/2027

This form is to register children Grade 2 or above (as of September 2026) to prepare for the Sacraments of First Reconciliation and First Communion.

To register for this program, please ensure that:
- the family are registered parishioners and/or live within the boundary of Our Lady Queen of the World Parish.
- the candidate have received the Sacrament of Baptism.
If any of the above are not met, please contact olqwsacraments@archtoronto.org prior to completing this registration form.

All sessions are mandatory and will take place on Saturday mornings, once a month, beginning in September 2026, in addition to several weekend Masses. Parents are expected to attend all sessions with their children. All sessions are in-person at the church.

*March 20/21, 2027 (any Sunday Mass) - Palm Sunday Procession
April 10, 2027 (10AM-12 Noon)
May 1, 2027 (10AM-12 Noon)
May 29, 2027 (5PM mass) - Corpus Christi Procession

First communion will take place on May 8, 2027.

The cost of the program is $100.
Refunds can be requested prior to the start of the program. NO refunds after program has begun. All refunds are subjected to 2% processing fee.

The deadline to register is: September 18, 2026.

A $100 payment is collected to help defray the costs of administering the formation program which includes all materials, and gown rental. The registration is not complete until payment is received.
