Confirmation Registration Form - 2026-2027

This form is to register young adults Grade 7 or above (as of September 2026) to prepare for the Sacrament of Confirmation.

To register for this program, please ensure that:
- the family are registered parishioners and/or live within the boundary of Our Lady Queen of the World Parish.
- the candidate have received the Sacrament of Baptism and Sacrament of Eucharist.
If any of the above are not met, please contact olqwsacraments@archtoronto.org prior to completing this registration form.

There will be a candidate and parent/guardian meeting on Wednesday, October 7, 2026 at 7:30 PM. We invite you to join us for mass at 7 PM prior to the meeting.

All sessions are mandatory and will take place on Wednesday evenings (7PM-9PM) from January 2027-May 2027, including some weekend masses. All sessions are in-person at the church and mandatory to attend.

Confirmation will take place on May 29, 2027 at 10:30 AM.

The cost of the program is $100.
Refunds can be requested prior to the start of the program. NO refunds after enrolment weekend on November 21/22, 2026. All refunds are subjected to 2% processing fee.

The deadline to register is: September 18, 2026.

Eligibility of Godparent (Confirmation Sponsor)

Each Confirmation Candidate should have a sponsor that meets the criteria below.

Sponsor information must be received by Wednesday, April 28, 2027.

Canon 892 Insofar as possible, there is to be a godparent for the person to be confirmed; the godparents is to take care that the confirmed person behaves as a true witness of Christ and faithfully fulfills the obligations inherent in this sacrament.

Canon 893 S1 To perform the function of godparent, a person must fulfill the conditions mentioned in canon 874 S1 (see below).

The following are the requirements in order for a Catholic to be a godparent (canon 874 S1):
- at least 16 years of age
- he/she has been fully initiated in the Catholic Church (received Baptism, Holy Communion, and Confirmation)
- in good standing with the Catholic Church: live a life of faith which befits the role to be undertaken; not under canonical penalty
- not the father or mother of the one to be confirmed

To receive Confirmation, the child must also have received First Reconciliation and First Communion. If they have not received their First Reconciliation/First Communion, they will have to enroll in the First Reconciliation/First Communion program at the same time.
